Ontrak Health will help Community Care Plan care for members by providing outreach, coaching, and much-needed behavioral health support and access to our members. We are thrilled to deliver these innovative solutions to our Medicaid members.”

She can easily attend the Ontrak sessions through telehealth. This has allowed her to address her issues of anxiety and control issues that had been negatively affecting her functioning in relationships. After 8 sessions, she has successfully met her treatment goals and has shared how this has positively affected her wellbeing as well as her family’s wellbeing.”
